We are a locally owned and operated home health care agency that is dedicated to fulfilling the health care needs of the patients of Fairfield County and surrounding areas. Phase 2 Face Home Care, LLC works closely with family members, hospitals, physicians and other members of the medical team to develop a care plan that will best meet the needs of the patient. You, your family and our staff will work together for a common goal while receiving face-to-face treatment throughout your phases of care… the best enhanced quality care possible in your home.

Our employees are competent, caring, and well-trained individuals who are responsive to the needs of our patients, their families, and the communities we serve. Our team includes, nurses, physical/occupational therapists, speech therapists, behavioral health nursing, medical social workers and home health aides.

Post-Surgical/Medical Care Services
Our services include, but are not limited to:
- Advanced Wound Care
- Enhanced Diabetic Management & Medication Administration
- Ostomy care (i.e. catheters, ileostomies etc.)
- Disease management & prevention from complications of many diseases such as hypertension, coronary heart disease or congestive heart failure.
Physical Support
Physical disabilities and age-related conditions are some of the few circumstances that prevent patients from caring for themselves.
Our caregivers can assist you with daily tasks such as dieting, exercising, strength training and use of assistive devices.
